	These services will be provided for the plan sponsor and plan participants by EMJAY Retirement Plan Services, Inc.:

	The client (and Financial Advisor) will be informed of issues that affect the plan EMJAY Retirement Plan Services, Inc. will contact the client (and Financial Advisor) with periodic updates once the plan is set up The client (and Financial Advisor) will be informed as to legal changes regarding plans including IRS required updates to plan documents Signature ready Form 5500 and Schedule B (the actuarial certification) will be completed as required each year Annual contributions will be calculated and communicated Forms will be prepared for participant distributions Forms will be prepared for loans
